PLEASE NOTE: Despite the fact that the agreement provides that eligible members may take job-protected unpaid leaves of absence with full benefits for the fourth quarter “upon request,” CPS states that applications for an unpaid leave of absence must be submitted via this form by 11:59p.m. on Monday, April 19. You will receive a confirmation email once your leave has been processed. If CPS takes down the form, you can still apply for an unpaid leave of absence after April 19 by sending an email to cpsloa@cps.edu.

Who qualifies? Options/alternatives if denied? If you have a high-risk condition as defined by the CDC, and are unable to be vaccinated for medical or religious reasons, CPS is required to grant your accommodation under the agreement. If denied, please inform your principal that CPS announced that you have this right under the new agreement and ask them to call HR. Then contact your CTU field rep. You can file a grievance and, if necessary, unpaid leave while awaiting your proper accommodation is an option. If you are the primary caretaker for a relative with a high-risk condition as defined by the CDC, and that relative is not fully vaccinated, CPS is required to grant your accommodation under the agreement. If denied, please inform your principal that CPS announced that you have this right under the new agreement and ask them to call HR. Then contact your CTU field rep. Unpaid, job-protected leave with benefits Employees not granted telework accommodations related to childcare will be offered alternate accommodaiton whereby their children may attend their regular school on all days when students are in attendance or a CPS Learniing Hub, subject to availability and eligibility.

High school employees who applied by April 5 for childcare accommodations but who were not approved may take a unpaid, job-protected leave with full benefits, regardless of their vaccination status.

FMLA leave (can use sick days while on FMLA leave), if eligible.
If ineligible for FMLA leave, apply for an unprotected, unpaid discretionary leave.
